2024-25 Athletics Handbook

Below are copies of various Athletic forms:

• Alternate Transportation Request Form - Required if you need to transport your child to or from an athletic contest
   
• "Blue Form" the SHS physical form
  All students participating in athletics must have a physical completed within the past 12 months and signed by a physician on file with the Health Office
   
• Petition Process for Reinstatement to the Athletics Program - for sports contract violations

 

Below are copies of the registration forms included in the Sports Registration Form
 
Please note these are for your information, you do not need to turn them in. Parents/guardians and students sign and submit these forms in the electronic sport registration.
