- Alternate Transportation Request Form - Required if you need to transport your child to or from an athletic contest
- "Blue Form" the SHS physical form
All students participating in athletics must have a physical completed within the past 12 months and signed by a physician on file with the Health Office
- Petition Process for Reinstatement to the Athletics Program - for sports contract violations

Please note these are for your information, you do not need to turn them in. Parents/guardians and students sign and submit these forms in the electronic sport registration.

Cautionary Statements
The Connecticut Interscholastic Athletic Conference (CIAC) has prepared sport-specific cautionary statements to inform parents and participants in high school sports about the inherent dangers and physical risks associated with playing sports. Each statement includes warnings and guidelines tailored to specific sports. We kindly ask that the student-athletes and their parent(s)/guardian(s) review and sign the cautionary statement for their respective winter sport.
