Alternate Transportation Request Form - Required if you need to transport your child to or from an athletic contest
My child is COVID-19 positive, what do I do now?
1) Isolate for at least 5 days.
2) Contact a medical provider for clearance to return to athletics. The day that you first experience symptoms or test positive if asymptomatic is Day 0 and Day 1 is the following day. Day 6 is the first day that an athlete could be cleared to return by a medical professional for any kind of activity (including RTP protocol).
a. Note from medical provider must specify that “AAP protocol has been incorporated in clearing the student-athlete to return to all athletic activities, including competition.” If the note does not specify this, the Athletic Trainer will assume that you are cleared to enter the Return to Play protocol (recommended by the American Academy of Pediatrics) which is a 7-day process.
3) Correctly wear a mask around others for an additional 5 days. Do not engage in athletic activities within 6 ft of others during which wearing of the mask is not advised due to safety reasons (i.e. wrestling) for a full 10 days.
